HEALTH bosses in East Lancashire are looking for residents willing to share their experiences and ideas to help provide better ‘joined up’ health and social care.

The NHS East Lancashire Clinical Commissioning Group would like to speak to over-65s who have received care in hospital or at home from health or social care services.

It also wants to hear from people with long-term conditions that they have had for more than a year and impact on their lives, such as heart disease, asthma, epilepsy, arthritis or mental health problems.

Dr Mike Ions, chief clinical officer said: “The CCG is committed to providing better joined up health and social care for patients in East Lancashire. Integrated care is making sure that people have the right support to live healthy lives in their communities, and have fewer stays in hospitals and care homes.

“We are planning to bring together local teams of people from a range of disciplines, such as nursing, social work, and physiotherapy.”
